The annual membership fee is based on an individual, age 7 and up. Payment plan is available with a credit card.

To sign up as a member of St. Ann, you must registered and listed in the church directory.
There is a one-time fee of $50 (individual) or $75 (family) to obtain a Health Card. If you are not a member of St. Ann, please sign up here.
You do not have to be a member of St. Ann, in order to join the Membership Care Program. The membership is for everyone.
